FOX HOUSE SUMMER CAMP WAIVER AND RELEASE OF LIABILITY
For Participants Under the Age of 18
IN CONSIDERATION OF the risk of injury while participating in Martial Arts and Summer Camp Activities (hereinafter the "Activity"), and in consideration of my desire and/or the desire of the minor child named below to participate in said Activity, I hereby acknowledge and agree to the following terms:
1. ASSUMPTION OF RISK AND RELEASE OF LIABILITY
I, {name}, the undersigned, am the parent or legal guardian of the participant (the "Releasor"), and I voluntarily enter into this Waiver and Release of Liability on behalf of myself and my child.
I understand that participation in martial arts, games, movement-based activities, crafts, structured downtime (including movies), and all other summer camp programming at Fox House Martial Arts involves inherent risks, including but not limited to: physical or psychological injury, illness, allergic reactions, emotional distress, property damage, and even death. These risks may arise from my child’s own actions or inactions, the actions or inactions of others, the condition of the facilities, equipment, or travel to and from activities.
I voluntarily assume all risks, known or unknown, and agree to release and discharge Fox House Martial Arts, Bang Muay Thai, Duane Bang Inc., their owners, managers, members, employees, volunteers, contractors, successors, and affiliates (the "Releasees") from any and all liability, claims, demands, or causes of action arising from my child’s participation in the Activity.
2. MEDICAL CONSENT AND RESPONSIBILITY
I authorize Fox House Martial Arts staff to provide emergency medical care deemed necessary, including but not limited to first aid, CPR, the use of AEDs, emergency transport, and the sharing of medical information with emergency personnel. I understand that I am financially responsible for all costs incurred as a result of such treatment. I have disclosed any relevant medical conditions, medications, and allergies in a separate health form or to the camp director.
3. BEHAVIORAL POLICY
I understand that my child is expected to follow all camp rules and demonstrate respectful behavior toward instructors, staff, and fellow campers. I acknowledge that Fox House Martial Arts reserves the right to dismiss any participant for unsafe or disruptive behavior, without refund.
4. INDEMNIFICATION
I agree to indemnify and hold harmless the Releasees from any claims, liabilities, damages, or expenses (including attorneys’ fees) arising from my child’s participation, whether caused by negligence or otherwise.
5. PROPERTY DAMAGE
In the event of damage to the facility, equipment, or another person’s property caused by my child’s intentional actions or recklessness, I agree to be held financially responsible.
6. PHOTO/MEDIA RELEASE
Unless otherwise indicated in writing, I grant permission for Fox House Martial Arts to photograph or record my child during the Activity for promotional or educational purposes, including social media and website use.
7. GOVERNING LAW
This agreement shall be governed by the laws of the State of Texas. If any portion is held invalid, the remainder shall remain in full force and effect.
